* Children must be 18 years old or younger or full time student.

* Family constitutes member + spouse and dependent children.

Because excellent primary care is built on the long-term relationship between doctor and patient, we request 6 months commitment to join our practice. We request 30 days notice, in writing, of cancellation. If cancelling membership you may re-enroll if the practice is accepting new patients, and subject to a $200 re-enrollment fee.
