DIRECT PRIMARY CARE
membership pricing
Enrollment fee of $100 per individual and $200 per family
* Children must be 18 years old or younger or full time student.
* Family constitutes member + spouse and dependent children.

Because excellent primary care is built on the long-term relationship between doctor and patient, we request 6 months commitment to join our practice. We request 30 days notice, in writing, of cancellation. If cancelling membership you may re-enroll if the practice is accepting new patients, and subject to a $200 re-enrollment fee.
As of January 2026, Dr. Angela is accepting limited new patients:
Men, family members of current patients and any patient who receiveds Mediare or Medicaid (BadgerCare) benefits.
Dr. Anne is accepting women and children into her DPC practice.
OSTEOPATHIC MANUAL MEDICINE
We are currently offering treatment only for direct primary care members and infants struggling with feeding. Consider becoming a member to benefit from discounted osteopathy.
